Before your next appointment

We look forward to seeing you soon and want to remind you that infection control has always been a top priority for our practice to keep you safe and comfortable during your visit. Because of this, please fully read the following information in preparation for your upcoming appointment:

Review the following list of screening questions and if you answer “YES” to any of them, call our office before your appointment for further discussion before your arrival:

  1. Have you been diagnosed positive for the Covid-19 virus at any time?
  2. Are you currently awaiting the results of a Covid-19 test?
  3. Have you been exposed to anyone who has been diagnosed with 
  4. Covid-19 in the past 14 days?
  1. Do you have a fever?
  2. Do you have any new shortness of breath?
  3. Do you have a dry cough?
  4. Do you have a runny nose?
  5. Do you have a sore throat?
  6. Do you have any sneezing, watery eyes, and/or sinus pain/pressure 

 that is unusual and not related to seasonal allergies?

  1. Have you experienced any new headaches, fatigue, or weakness?
  2. Have you lost your sense of taste and/or smell?
  3. Have you visited or received treatment in a hospital, long-term facility,

 or other healthcare facility in the past 30 days?

  1. Within the last 14 days, have you, or anyone in your household   

 traveled within the United States or to any foreign country?

  • If so, please call to inform us of the location, your mode of transportation, and how many days it has been since the travel occurred.

Our office follows recommendations made by the Illinois Department of Public Health (IDPH), the American Dental Association (ADA), the U.S. Centers for Disease Control and Prevention (CDC), and the Occupational Safety and Health Administration (OSHA). We follow the activities of these agencies so that we are up to date on any new recommendations or guidance that may be issued. Because of this, we have also implemented the following protocols:

  • When you arrive for your appointment, please call the office at (309) 691-1990 to make us aware of your arrival.  Please then wait in your car, and we will call or text you when it is time for you to enter the building. For everyone’s safety and to respect the social distancing guidelines, we are minimizing the use of our reception area. 
  • Please wear your own mask upon your arrival to the office for use when you are not in a treatment room. You will see that we will be wearing our masks as well at all times.
  • We ask that you do not wear gloves in order to avoid cross contamination. Common used surfaces are wiped on a regular basis through the day and each treatment operatory has been thoroughly disinfected for your appointment. For your safety and convenience, hand sanitizer is readily available throughout the facility.
  • Your provider will meet you at the door, provide hand sanitizer, and take your temperature. We will review with you the screening questions listed above and there will be an informed consent for you to complete.
  • If all is well, we will proceed to the treatment room where we can update your medical history and proceed with your appointment. 
  • We are unable to provide our patient blankets at this time, so if you are prone to feeling chilly, please bring along a sweater or jacket.
  • We require that you come to your appointment alone, unless you are a parent of the child whom has the appointment or you require a caretaker to assist you.
  • Unfortunately, we no longer have magazines, but we still provide free Wi-fi.

Our community has been through a lot this year, but we are all doing our best to get used to the new normal while resuming our regular appointments and daily routines. While many things have changed, one thing has remained the same: our commitment to your dental health, safety, and well-being.
